Indoor Playground Liability Waiver & Parent Consent Form

Assumption of Risk

I understand and acknowledge that indoor playground activities involve physical activity and inherent risks. These risks may include, but are not limited to, slipping, falling, collisions, equipment-related injuries, and interactions with other children. I voluntarily accept these risks on behalf of my child(ren).

Release of Liability

In consideration of being permitted to participate at CandyLand Indoor Play Centre, I hereby release and hold harmless CandyLand Indoor Play Centre, its owners, employees, contractors, affiliates, and staff from any and all liability for injuries, damages, losses, or claims arising from participation, except where prohibited by law. I acknowledge that participation is voluntary and that parents/guardians remain completely responsible for supervising their children where applicable.

Health & Medical Authorization

I confirm that the listed child(ren) is/are physically able to participate in indoor playground activities. I have disclosed any relevant medical conditions or allergies in this form. In the event of an emergency, I authorize staff to seek emergency medical treatment if required, and I accept full responsibility for any medical expenses incurred.

Playground Rules & Safety Agreement

• Socks (and specifically grip socks if required by facility) must be worn at all times in the play area.
• Children must follow all playground rules and staff instructions.
• Unsafe or disruptive behavior may result in immediate removal from the facility without refund.
• Parents/guardians are fully responsible for actively monitoring their younger children.
• The facility is not responsible for lost, stolen, or damaged personal belongings.

Illness Policy

Children showing signs of illness (including fever, heavy cough, runny nose, or rash) should not enter the facility. Management reserves the right to refuse entry for safety reasons. I confirm that my child(ren) is/are not knowingly sick with contagious symptoms.
